Position Overview

The Bilingual Case Worker will serve as a vital pillar in the Family Support Program within the Outreach and Community Initiatives portfolio at Sacred Heart Center. This position will provide individualized, culturally-affirming case management to improve the lives of Latino individuals by assessing their needs, providing community resources information, and providing support and advocacy in navigating care and support services systems. The Bilingual Case Worker will work very closely with the other members of the Family Support Program team n a coordinated care model to assist Latino individuals in overcoming obstacles and reaching their goals.

Key Responsibilities

The Bilingual Case Worker will have the following primary responsibilities:

  • Conduct in-person and phone outreach, screening, and intake to determine eligibility in the program
  • Assess family’s needs and barriers and identify internal and external resources and services that are consistent with each family’s identified needs
  • Assist the participants in brainstorming action steps and developing a plan of action using a strength-based approach to help them reach their self-identified goals
  • Serve as a liaison between the client and social, financial, health and legal services
  • Provide follow-up phone calls or in person visits at SHC to assess progress and additional needs
  • Maintain detailed case notes about all client interactions and referrals and protect client confidentiality
  • Discuss shared cases with the Family Support Program team to ensure clear communication, collaboration, and coordination of care
  • Maintain and build upon existing community partnerships and foster relationship-building between agencies

Other tasks

● Provide support to other Case Worker-Navigators in the Family Support Program team

● Update the Sacred Heart Center Resource Directory to add new resources and ensure that

existing resource information is up to date

● Provide training to other team members and/or social work interns if needed

● Collaborate at Sacred Heart Center events and with other Sacred Heart Center programs as

Experience and Attributes

Ideal candidates for this position will share our commitment to our mission and to our values of compassion, inclusion, equity, and collaboration, and have related experience, including:

  • Bilingual in Spanish and English required. Bicultural preferred.
  • Bachelor’s or Associate’s Degree in Social Work and a minimum of two years’ experience in case management in a human service field and proven experience working and advocating for Latinx families in the Metro Richmond area.
  • Highly organized, analytical, IT-competent proficient in Microsoft Office programs, the Google work suite, Apricot case management software, and ability to learn new programs as needed.
  • Ability to organize and maintain detailed records, including case notes, complete necessary paperwork, and meet deadlines.
  • Exemplary record of strong communication and interpersonal skills and customer service focus, excellent follow-up, and an ability to work with individuals with diverse backgrounds and experiences in a courteous and professional manner.
  • Ability to connect with others and forge strong relationships.
  • Personal qualities of integrity, credibility, and a commitment to and passion for SHC’s mission and values.
